Performance Appraisals.

Hope you have gone through Gomez book on the topic of performance management...
Did you observe how your organization is doing mistakes in the approval process?
Atleast this is my impression from my organization:
Most of my performance appraisals happened thru bias..hardly ever through achievements.
Statistics: Only 2 out of 8 projects I have worked on, are actually successful, seen the customer.
But, I am consistently rated as outstanding...
What are your views?
